Welcome to the National University of Natural Medicine (NUNM) Health Center!  We are delighted that you consider us for your health care needs. Our team of doctors, providers and students offer the best in both traditional and integrative naturopathic and Chinese Medicine. We are pleased to offer our services as Primary Care Providers (PCP) in our Patient-Centered Primary Care Home (PCPCH).

What to Expect During Your Visit

As we are a teaching institution, you will be attended first by our Primary Intern students.  These are our senior Naturopathic or Chinese medical students.  Your student Interns will be devoted to your medical concerns, and serve as your main contact with the supervising Provider.  Your student Interns are responsible for gathering your data, discussing your medical history, collecting vital measurements of your health, and holding space to hear your concerns about your conditions and potential treatment plans.  The primary Intern will then discuss your case with their overseeing Provider and Resident team. 

Next, the supervising Provider and/or shift Resident will join you in the treatment room to discuss possible diagnosis, lab testing or referral needs, and options for treatment.  We pride ourselves on building collaborative care plans while acknowledging the potential challenges you may face.

Forms to Complete

Registration Packets

If you’re a New patient to NUNM, or if we haven’t seen you in more than 3 years, we will need a complete and updated patient registration packet.  This packet includes required demographic and insurance information as well as a comprehensive health history.  These documents must be completed BEFORE we can schedule your appointment. Please allow 5 business days to process your application. 

The Registration form contains space to attach information about your health history and medications.  You will also be asked to provide photos of your insurance card and ID.  These photos can be taken by phone camera or scanned and attached to the document.  This form contains required fields which are outlined in red.  The form can’t be submitted until all required fields are filled.    

Consents For Care

NUNM Consents will need to be signed before your first visit and once a year thereafter.  You can review these documents below, and are welcome to complete them online before your first appointment.  If we do not receive these signed documents before your appointment, they will be made available at the front desk to sign at your check-in. 

Other Health Center Documents

To accommodate patients with no insurance, or with non-contracted insurance, NUNM offers our Compassionate Care Program, a sliding scale discount on office visits and some other forms of care.  Below is the application for our Compassionate Care Program.  This application must be renewed yearly.  We require proof of income such as a tax return, paystub, or benefit letter in order to offer services at a discounted rate.
