Welcome! The NUNM Health Centers team is honored to provide you with the care you need and deserve. We commit to honor the trust that you have put within our providers, students, and staff by providing you the best natural care.

What to Expect When You Visit

The student interns on shift with your provider will review your health history, do vitals such as height and weight measurements, and ask you questions around your health priorities to determine a care plan. It is often helpful to have copies of previous health records in hand, or faxed to our medical records team in advance of your appointment.

At your first appointment, you meet your team of providers – typically two students, an attending practitioner and sometimes a resident physician. Your primary contact and communication will be with your “primary” or “intern,” a senior naturopathic or Chinese medical student. The students will talk with you about your current health concerns and your health history. They then confer with the supervising practitioner on the next steps – physical exam, further history, lab work, etc. You are offered information and choices during all phases of building a treatment plan for you.

Forms to Fill Out

NUNM patients will need to complete a number of forms to become an established patient. Please first call our scheduling line so that we can ensure you are aware of your options.

English language forms can be completed electronically by clicking the links below. You will be taken to a webpage hosted by SignNow where you can complete the form online and securely submit by clicking the Done button in the upper right corner. You may also download a digital copy of the form if you wish to complete the form manually.

Spanish language forms can be completed by downloading the form via the hyperlinks provided. We are hard at work to set up digital submission for Spanish forms – coming soon!

After forms are submitted electronically, it may take a few days to process your paperwork. We appreciate your patience and will contact you when we are ready to schedule your first appointment!

Registration Packets

If you are a brand new patient to NUNM or if we haven’t seen you in more than 3 years, we will need you to complete a registration packet in full. These packets contain all of the necessary forms to get established as a patient.

Consents For Care

These individual consents are already included in the New Patient Packets. You only need to complete these forms if they have been updated, expired, or your information has changed.
