Lab orders are only accepted via fax or directly from patients. Please fax all orders to: 503.257.5929. Orders called in will not be accepted. Unsigned orders will not be accepted.
The following information must be on all lab orders and must be legible.
- Date
- Patient’s full legal name
- Patient’s date of birth
- Provider’s name
- Provider’s contact information
- ICD-10 codes
- Tests required
