How to Order Labs

Lab orders are only accepted via fax or directly from patients. Please fax all orders to: 503.257.5929.

Orders called in will not be accepted. Unsigned orders will not be accepted.

The following information must be on all lab orders and must be legible:

• Date
• Patient’s full legal name
• Patient’s date of birth
• Provider’s name
• Provider’s contact information
• ICD-10 codes
• Tests required
